New: Cooperative Healthy Savings Plus

In the new year, South River EMC is offering members the chance to participate in a program called Cooperative Healthy Savings (CHS).

CHS is a program that costs $4.25 a month, but gives members discounts on dental visits, as well as hearing and vision equipment.
The program provides you and your family with a way to save money on basic medical care, products and services at prices that meet your budget.

• Save 10-60% on prescription eyeglasses and contacts and participating retailers. Save 10-30% on LASIK surgery. Save 10-40% on mail order contacts.
• Save 15-50% on general dentistry and cleanings.
• Save 15% on Beltone hearing aids or save 37-58% on hearing aids
  

Upon payment of the plan, you will receive a membership kit, which includes your card and booklet containing benefit and provider information. This information shows the providers closest to your home and you can locate more providers by calling the toll free number on the back of your card or searching online at www.locateproviders.com.

The CHS card is not insurance and should not be used as insurance. You can sign up for the plan if you have insurance. With rising deductibles and gaps in coverage, it may make financial sense for you to use your card to help minimize out- of- pocket expenses that are so common with health plans.
Your card is good for you, your spouse, and all legal dependents.

This program has a 30-day-cancellation period.
This plan provides discounts at certain healthcare providers for medical services.
The plan member is obligated to pay for all healthcare services but will receive a discount from those healthcare providers who have contracted with the discount plan organization.

The Cooperative Healthy Savings Plus programs, offers the same great benefits as Cooperative Healthy Savings, with an addition. Consult-A-Doctor, which is available to participants 24 hours a day, 7 days a week. Physiucians can discuss symptoms, recommend treatment options and prescribe medication when appropriate.This prgram costs $8.95 a month.
